If you would like to have your child baptized at St. Monica Church, please fill out the request form below. After you have completed the request, you must provide our parish office with a copy of your child's birth certificate and the Godparents Eligibility Form (which can be downloaded here). Once these documents are in order, the baptism will be scheduled.
Please be aware of the following requirements before making your request:
One of the parents must give evidence of practicing the Catholic faith.
Parents must be registered members of the parish (4 months) with exceptions made for grandchildren of our parishioners as long as the parents of the child present a letter from the pastor of their church where they worship and they meet all St. Monica's baptismal requirements.
Godparents must be verified by their parish, stating they are confirmed Catholics and in a sacramental marriage, if married.
Parents and Godparents must participate in baptismal classes at St. Monica or their own parish.
Both parents must attend an interview with Fr. Paul Coleman, Parochial Administrator.
All documents (i.e. Godparent forms and copy of birth certificate) must be received in the office one month prior to scheduled baptism. If not received the baptism date may be rescheduled at Fr. Paul's discretion.
